What to expect
 It may seem like there are a lot of forms to complete, and you may feel that some of the requested health information is not relevant to your foot condition. Rest assured that each and every question is relevant to many foot conditions, and obtaining a thorough health history is an integral part of providing quality foot care.

New Patients are asked to arrive 30 minutes prior to their scheduled appointment to complete the registration process.
